Signature Staff Resources offers 3 Medical plans. All plans provide 100% in-network coverage for all preventive services required by the Affordable Care Act (ACA). The plans also include coverage for healthcare services due to an accident or illness. Should a discrepancy exist, plan and carrier documents and rates prevail
Health Insurance
- Requires an Individual health questionnaire
- Annual deductible – Individual $4k and Family at $8k in network
- 80/20% Share in Network
- Office visit – flat amount paid by insurance of $60
- Outpatient Diagnostic Lab – flat amount of $50
- Teledoc – 100% paid

Dental
- Deductible $20 per visit
- Preventive and Routine Exams – no waitingperiod, covered at 100%
- Major services like crowns – 12 monthswaiting period
- Calendar year maximum $500 per person
- Basic Services, filings composites – 3months waiting period
Vision
- Deductible – $10 per exam, per person
- Annual exam covered 100% in network, out of network up to $45
- Frequency – Frames every 24 months
- Deductible - $0 Eye Glass Lenses /15% discount Contacts / Up to $105Frames
- Frequency – Exam & Lens every 12months
Short Term Disability
- Coverage for Employee at $3.50 weekly, waiting period of 7 days
- Pays flat amount of $150 weekly up to 26 weeks
- Roth and Pre-Tax plans offered
- Employees can defer up to 98% of theirsalary
- Can choose either flat amount or percentagefor payroll deduction
